Note: After downloading a form and filling it out, please fax directly to: 1-888-810-3200. Please write down this fax number. You can also mail these forms back to us (see below).

Do NOT fax to the number printed on the form. We (your agent for AllCom here at CTS, Inc of VA) need to process these forms in our office prior to submitting them to AllCom so that you get the lowest rate available from AllCom -- we need to add the rate quote for the AllCom plan we have quoted so you get AllCom's lowest available rate. AllCom does not solicit long distance service directly with any customer -- they only receive customers through their team of authorized independent agents. We also do a thorough check of the form to make sure the form will be processed with the least amount of error and in the quickest amount of time. This is part of the free service we offer to you as your agent.

ByPass Plan Letter Of Agreement (LOA) form allows you to use AllCom without switching your long distance service. After submitting the form, to use this service, you dial a toll-free number, then dial your long distance number and you are connected. No switching to AllCom service is required. Can be used to check the quality of service AllCom offers before actually switching your long distance service to AllCom, or is sometimes used if you are currently paying a high rate for your in-state (intrastate) rates as AllCom's intrastate rates are usually considerably lower than most long distance companies. Also, international calling using the ByPass is usually much less. Note that there are no monthly fees or hidden charges, but there is a low usage fee of $2.50 for accounts that bill less than $20/month:

Residential Letter of Agreement (LOA) form is used to start new long distance service for outgoing (1+) calling service, new incoming (800/888/877/866 toll-free), and calling card(s) for a residence: (for switching an existing toll-free number that you have over to AllCom, download the Toll-Free RESPORG form -- see below)

Business Letter of Agreement (LOA) form is used to start new long distance service for outgoing (1+) calling service, new incoming (800/888/877/866 toll-free), and calling card(s) for a business:

Toll-Free Transfer (RESPORG) form is used to transfer an existing toll-free number  from your old long distance company to AllCom; can be used for either residential or business toll-free transfers:

Monthly Bank Draft form is used to automatically pay your long distance bill from your checking account:

Monthly Credit Card Draft form is used to automatically pay your long distance bill by credit card. Saves you postage and time for having to fill out a check. Very convenient:
