Two forms are available:
Residential or Business Letter of Agreement (LOA)
800 RESPORG (for transferring an existing toll-free number; not required for ordering new toll-free #'s)
If needed, second page of Letter of Agreement (for adding more than 6 phone numbers)
Residential or Business Letter of Agreement (LOA) form is used to start new long distance service for outgoing (1+) calling service, new incoming (800/888/877/866 toll-free), and calling card(s) for a residence or business.
