Four forms are available. All forms should be faxed directly to Agent, not to MetTel. The toll-free Agent's fax number is listed at the end of this web page. The number of seconds to download each form is in fuchsia color after the "Click Here" button.

Letter of Authorization form
Used to authorize MetTel to obtain billing information, customer service records, and other network information to make the change in service.

Letter of Agency form
Used to detail the information on obtaining service, and includes customer satisfaction agreement and what MetTel will do. Basically, if you as the customer are not completely satisfied during the first 60 days of service, you may terminate the agreement and MetTel will reimburse or credit you the cost of switching to a different carrier (and additional contractual information -- read the form for details).

Toll-free RESPORG form
Used to transfer an existing toll-free number to MetTel's service. Used if you have a Bundled Basic and Bundled Extended service. This form cannot be used if you select one of the other long distance companies (not MetTel's service). The other long distance companies have their own form -- call us to obtain these other forms.

Monthly Local Service Comparison
If you wish to see how much you can save monthly, annually, and in 2 years by using MetTel, download this form. This simple form asks for your Billing Telephone Number (BTN), your current monthly billing, the basic rate you are paying for each line, and the dollar amount for local, regional, and long distance calls. All this information can be obtained from your Verizon local monthly phone bill and your current long distance carrier's monthly bill. You will get a side-by-side comparison of these items with MetTel's bill. To insure quickest service, please fax back to your Agent at 1-888-810-3200. If you have any questions, give us (your Agent) a call at 1-800-267-8276. Do not use the phone number or the web site address printed on the form.
